At some point in the game Falcone will ask you to take care for a special task, which will be to drive a woman to a specific house.
He will give you the car keys, so after that you will be able to go on the map to a place called "Drugstore".
Just go there and achievement will pop up.

Story related, cannot be missed.

This trophy is at the Drugstore and will unlock by arriving there for the first time. You'll have to fight 2 opponents so make sure you're fighting smart and play it safe. Be sure to use first aid tokens to heal as your health carries over into the next fight.
