This achievement is one of the easier ones to obtain.
All you need to do is load up a game ( I'd suggest expert as there is more space to play around with) and start solving the field. Once you come across the pattern 1 - 2 -1 you must quickly uncover the correct tile and marked mines within 3 seconds of revealing it. Normally that will be done upon instinct.
Once you've done that remember where the pattern is on the board and blow yourself up. You can then replay the same board and go to the same 1 - 2 - 1 pattern and do it again. Then all you need to do is rinse and repeat that until you unlock the achievement. Easy!

Note: unlike some of the other cumulative achievements in this game, this one doesn't track. It'll stay at 'locked' until you have 20 1-2-1 clears.

Also this method doesn't seem to work for the 1-2-2-1 achievement. :(
