**Story Based Achievement**
This cannot be missed, just complete mission 10 of 20 and this will unlock upon completion.

Mission 10 complete and no achievement popped :-(
EDIT: 2 more playthroughs to where this achievement is supposed to pop on separate save files and still no achievement.
Fully reset Series X (keeping games), redownloaded my profile, deleted persistent storage and fully deleted ALL my console and cloud saves for this game (deleting my elite scores for the Tests which I wasn't happy about); I also moved the game installation from my spare official SSD (the one that plugs in at the back) to the internal SSD; restarted again for a FOURTH time and the achievement unlocked today. Really spoilt my impression of this game. Am hoping Shadow Of War isn't also riddled with achievement bugs...
One thing I did differently was that when I died I didn't dashboard to keep the Nemesis system intact and more importantly I NEVER ever quit the game mid mission. I always made sure missions were completed before I quit any session. HTH anyone getting other glitched achievements aside from this one. Good luck!
