After an uruk kills you to become a Captain, help him become a Warchief, then kill him.

How to unlock the Rise and Fall achievement in Middle-earth: Shadow of Mordor - Game of the Year Edition - Definitive Guide
This is best done once you have acquired the ability to brand uruks (second area of the game). It is then quite easy.
Let a random Uruk kill you, he will then be promoted to captain. After this make him your target and then brand him so you control him. Then command him to become a bodyguard and ensure he completes the test. Then dominate him again and order him to betray his war chief. Ensure he passes this test by killing the war chief. Immediately after your Uruk is promoted to war chief press down on the d-pad to kill him and the achievement should pop.
UPDATES:
1. A couple of people (see comments) have said that dispatching the uruk (pressing down on the d-pad) didn't pop the cheevo for them. It worked for me so there may be some quirks where it does/doesn't work.
2. It seems you cannot have him kill/betray a branded warchief (or captain for that matter). Thanks to Warboy925 and mantledFeanorNL for pointing this out.

Regarding the fact that you can't have them betray or kill a branded warchief: you can still summon your branded warchief and the bodyguard and then manually kill the chief. Then kill the promoted bodyguard. Worked for me

I did the whole thing, my boi became Warchief. I immediately kept hitting him, ended up shooting him with arrows, he died & I got nothing
