Survivalist challenges are completed by harvesting certain herbs.
The main map shows you on the right hand side what herbs are required and the herbs are visible on your mini map on the bottom left of the screen.
If you're struggling to see them, activating Wraith Vision (LB) causes herbs to be highlighted.

This is actually fairly easy. If you open your map with the touch pad, you can see on the right side you'll need to collect 2 Niphredil. These are herbs, collected with R1, scattered throughout the game. They are shown only on the mini map and can be identified by the four white arrows, just like it is shown on the map next to the tracker.
