This is a story-related achievement and cannot be missed. It is unlocked for completing mission "The Warchief."
You will be tasked to kill warchief Mogg the Other Twin for Ratbag. See the Power Vacuum achievement (and solution by PBreezey as well), as this is a good time to get that achievement:

I'm not certain if Mogg is randomly-generated like the other warchiefs. In my instance, to get him to show I had to kill 2 berserkers - this was very easy as you can lure them to the edge and stealth kill them. Mogg himself was vulnerable to both ranged attacks and combat finishers and therefore an easy win.

The achievement is story related, but it doesn't drop. Also, the achievement to save the life of Lithariel does not drop, all two story achievements, but they do not drop, tested on XBOX ONE and XBOX ONE X.
