Story related, impossibile to miss if you're going to beat the game.
This is the second to last mission in the story: after you've branded all 5 Warchiefs in the 2nd map you can reach this final location.
First of all you'll have to win a battle against 5 mini-boss (same strenght of normal Warchiefs), but you have your branded guys with you, so you'll beat them in a snap.
The final battle is against The Tower. This can seems tricky at first, but actually you just have to be carefull: you cannot face the Tower directly, or he will brand you and kill you in 3/4 times (depending on how hight is your health bar).
What you have to do is to sneak behind him and brand him. The Tower is present in multiple avatars, when you brand one of them the avatar will disappear.
Use ghost vision to seek him out, then have a look to his path. The Tower usually stands and turn his head after a few steps; so keep covered behind a pillar and take your time. When he starts moving, just reach him and brand him with

Do it 3 times and you'll got it.

Has anyone ran into a glitch with this achievement? The achievement itself says, "Done! Unlocking..." However it didn't unlock. Yet I did get The Hand is Severed achievement.
I am thinking I am out of luck but if if there is a solution, it would be greatly appreciated. Thanks!

This has glitched on me including several others as well,damn why is game so broken.

Story related. Cannot be missed.
To unlock this trophy you must complete the main story mission “Lord of Mordor”. After successfully branding all 5 Warchiefs in the “Brand the Warchiefs” mission, and rescuing Lithariel from the orcs in “The Rescue” mission, you can finally make your way to The Black Hand. You will meet up with the Queen, Lithariel and a ship captain that will take you across the sea to the heart of Mordor.
Upon reaching the shore you and your five Warchiefs will battle with a captain and roughly 20 Uruk-hai. Defeat them all and make your way into the town to face the faux-boss, The Tower, not your intended target The Black Hand. A short cutscene will ensue and you will have to track down The Tower and use Stealth Drain (

) to to defeat him. Use Wraith Form (
) to track down his location and travel along high ground so that you are not seen.
After three stealth drains you will have to find your wife, followed by yet another cutscene. Once the cutscene ends you will unlock the trophy.
