The Best Defense Achievement/Trophy can be unlocked as early as Chapter 2. You will meet Bruz in Chapter 2 who will give you tutorials on how the domination system works to make captains allies and one of the tutorials will be the 'Siege Mode'. Basically what you will need to do is reach Level 20 and have a total of 2,250 Silver before this is do-able.
Once you have the above, recruit 2 captains and go to your Siege Tab in the nemesis menu. Now add the 2 captains to the empty spots under Bruz and beside the captain, will be 3 different possible upgrades for each captain. Each upgrade will cost 750 silver and will add a significant bonus to your assault on a fort later on. After you purchase an upgrade for each of the 3 siege leaders you will get the achievement/trophy.

From this point, you will need to dominate two more captains (in addition to Bruz). You can assign the captains in the siege tab as siege leaders and equip upgrades to them. Note that, you will need to be at least level 20 to be able to make 3 siege upgrades.

To unlock this achievement you need to equip three siege upgrades to your assault team. This will only be possible later on into the game when you have to attack the fortresses to take them over. This is first done in Cirith Ungol. When you accept the quest to attack the fortress you will be presented with a screen that shows your assault team and the defending team.
On this screen press 
to manage your team and from here you can purchase more slots to assign more of your captains to the assault team as well as purchase and equip upgrades to each captain. You have to have at least three slots open each with an assigned captain, then to the right of their slot there will be two to three smaller slots, each of these is an upgrade. Only one can be applied to each slot at once so purchase one for each of your three captains and the achievement will unlock.

Siege upgrades are available in the range of 700-1105 Mirian each, you unlock more slots as Talion levels up and by completing Champion Pit Fights. These can be accessed on the Army screen by pressing
for siege upgrades, assigning someone from your recruited orcs to an available slot and then pressing 
to buy a siege upgrade. Do this on 3 separate slots to unlock the trophy.

1) Before starting the quest, you must have recruited at least 3 captains.
2) I don’t know about lvl 20, as in the hint above, I didn’t check, but my hero was lvl 21.
3) You must have 3000+ game currency, otherwise there will not be enough for upgrades for all captains.
4) So we have everything ready, we take the quest near the fortress with a white icon depicting a tower. To make it easier, you can recruit two captains, the leader’s guards.
5) As soon as you start the quest, on the left there will be a column with icons and one plot captain. We add one improvement out of three to the captain, then add the second and third captain, and one improvement for each. Every improvement costs money. We start the quest and get the trophy.
