it is also glitched for some people. I have him redominated and he is mentally broken, but still no achievement and the bruz quest list is at 100%

I have now completed his quest lines twice over on new saves and still haven't got it. I've been able to unlock all other original achievements and even a dlc one without owning the dlc, is anyone else having the same issue or know of a fix?
I've hard reset my Xbox, reset my connection, uninstalled and reinstalled the game.

Glitched. Completed all the quests, Hard Reset, still hasn't unlocked. It doesn't even say Done....Unlocking! It's still locked.

As soon as a start the Quest "Worse Than Death" the game crashes and throws me back to the Xbox Home screen

Is anyone else having this issue or know how to fix it?
