If you want to get the Top tier Gem, the first thing to do is:

In my opinion the easiest way to get the achievement is while playing "The Blade of Galadriel" DLC.
When you play the expansion you can drop only one type of gems from the enemies (Gems of light - it's a new gems existing only in this DLC). You do NOT even need to farm or grind them because they can be looted from Captains, Warchiefs and Overlords while playing the storyline, I was able to craft the top tier gem after completing the mission "Poisoned Ground" its like in the middle of the DLC's storyline.
I bet you can get it even faster if you do "Fallen Blades" side-missions because you also kill the captains there

If someone does not know how to upgrade the gems, you need to go into inventory, next choose any gem's slot and hold cn_X on the gems which you want to upgrade.

I hope it helps, good luck guys :)

To unlock this achievement you will need to forge a top tier gem. There are five levels of gem; basic/unnamed, Carved, Polished, Refined and Perfect. You need three of any level of gem to combine into one of the next tier. You can find gems throughout the game as loot for completing missions, secondary objectives and from killing treasure Orcs that have a white symbol above their heads. You will typically find the basic gems however once you have collected all of the Gondorian Artifacts you unlock a skill called 'Prospector' that gives you an increased chance of finding higher quality gems so make sure to activate this once you have unlocked it to make this achievement easier, or else you will mostly be relying on basic gems which means you would need eighty one basic gems to make a Perfect one.

Once you have a Perfect gem by combining previous ones to forge it the achievement will unlock.

For this trophy you need to forge the highest (5th) tier of gem, this will definitely take a while as you always need 3 gems of the previous tier in order to forge one of the next tier. Finding all Gondorian Artifacts unlocks the 'Prospector' skill, which makes all enemies/monuments drop higher quality gems which I highly recommended going out of your way for this.
When playing you can actively kill white highlighted enemies, which are fairly common in the game world to get plenty of gems, sometimes they will drop Mirian instead. Ologs will very rarely carry higher quality gems so keep on the look out for these, they show as a white arrow outlined in red on the map. You can also destroy monuments for a gem and getting gold ratings on Shadow of the Past missions will reward you with a Polished (3rd tier) gem.
If you don't get a gem of the highest tier before Act 4 you will be able to finish this off during the Shadow Wars, as plenty of gems will drop whilst you are defending your fortresses, especially if you are actively killing Graugs and Drakes in process.

Like the “Blacksmith” trophy, you just need to upgrade the gems to the maximum level.

It may be useful for someone who, like me, did not pay attention to gems and by the end of the game discovered that there were very few of them and before the trophy they still had to look for white orcs and look. There is an outpost in Nurnen where orcs spam from the barracks doors endlessly. Set the gold digger option in your skills (appears after collecting all the treasures) and kill the orcs, it took me about 10-15 minutes. For the lazy ones, you can burn them on the dragon, then collect all the gems.

If you have the Desolation of Mordor add-on, in the “About Shadows of the Past” quests from the area of this add-on (Litlad, sort of) high-level gems are given as a reward. If you go for gold, you will also receive gems of the highest level.
It’s very simple and, if you are going to get 100% in any case, it won’t take up extra time.

Don't rush to break statues. First collect all the artifacts. After this, a perk opens that gives the dropped stones a chance to become polished. After that, go break the statues. By the end of Act 2, I already had one stone of each type of maximum quality.
