How to unlock the Master Forger achievement in Middle-earth: Shadow of War - Definitive Guide
Find the caravan of orcs (white arrows on the minimap) sometimes killing them drop silver coins or gems.
Find Monuments and destroy them (monuments only drops gems)
Second thing to do is:
combine 3 gems of the same type in your inventary
Phase 1: x3 Normal Gems = 1 Carved Gem
Phase 2: x3 Carved Gems = 1 Polished Gem
Phase 3: x3 Polished Gems = 1 Refined Gem
Phase 4: x 3 Refined Gems = 1 Perfect Gem
Phase Final: Unlock your achievement
When you play the expansion you can drop only one type of gems from the enemies (Gems of light - it's a new gems existing only in this DLC). You do NOT even need to farm or grind them because they can be looted from Captains, Warchiefs and Overlords while playing the storyline, I was able to craft the top tier gem after completing the mission "Poisoned Ground" its like in the middle of the DLC's storyline.
I bet you can get it even faster if you do "Fallen Blades" side-missions because you also kill the captains there
If someone does not know how to upgrade the gems, you need to go into inventory, next choose any gem's slot and hold on the gems which you want to upgrade.

To unlock this achievement you will need to forge a top tier gem. There are five levels of gem; basic/unnamed, Carved, Polished, Refined and Perfect. You need three of any level of gem to combine into one of the next tier. You can find gems throughout the game as loot for completing missions, secondary objectives and from killing treasure Orcs that have a white symbol above their heads. You will typically find the basic gems however once you have collected all of the Gondorian Artifacts you unlock a skill called 'Prospector' that gives you an increased chance of finding higher quality gems so make sure to activate this once you have unlocked it to make this achievement easier, or else you will mostly be relying on basic gems which means you would need eighty one basic gems to make a Perfect one.
Once you have a Perfect gem by combining previous ones to forge it the achievement will unlock.
When playing you can actively kill white highlighted enemies, which are fairly common in the game world to get plenty of gems, sometimes they will drop Mirian instead. Ologs will very rarely carry higher quality gems so keep on the look out for these, they show as a white arrow outlined in red on the map. You can also destroy monuments for a gem and getting gold ratings on Shadow of the Past missions will reward you with a Polished (3rd tier) gem.
If you don't get a gem of the highest tier before Act 4 you will be able to finish this off during the Shadow Wars, as plenty of gems will drop whilst you are defending your fortresses, especially if you are actively killing Graugs and Drakes in process.
It’s very simple and, if you are going to get 100% in any case, it won’t take up extra time.
