Just an FYI, as I just completed the game on Realistic and you don't really need any tips since the game is fairly easy with all the checkpoints and such.... difficulties do not stack. You'll have to do at least two playthroughs to get the Hard and Realistic achievements.

Do i have to start a new game, or can i do it via level select?

You can do it via level select and you can use any unlocked challenge perks.

If I do all the levels on hard via level select will I unlock realistic difficulty

i'm on HARD... i got the cheevo for completing the game ... but not the Hard one... makes total sense right?

You can't use perks until you have completed the level atleast once. And also if you change difficulty you will have to complete the level again to use perks on that difficulty.
