While the achievement description is unnecessarily confusing, this is really quite simple. All you have to do is beat the level in under 9 minutes having used each transformation at least once. Make sure you actually hit an enemy or boss with each power as well. It's a pretty lenient time to beat if you know what to do, and Cryo/Bat/Brand are all you need for the bosses.
Here's a video of me completing the stage in 4:38 with an S rank. I don't use all transformations in this, but as you can tell by the time, there's plenty of room to fit that in and still comfortably hit the target.

Worth noting that you have to actually use abilities on enemies for it to count. I just went down the list and just transformed into each one for my first run and it didn't pop, second time around I made sure to kill an enemy with each and it did.

Ah thank you, I wasn't sure about that. I used all my abilities on the boss just to make sure when I did it.

In the level you are going to use: Battalion, Aviator, Brandish, Cryo, Seismic, Countershade, and Dyna to get past the obstacles and fight the bosses. You only have to make sure you use Pryo on an enemy in the beginning and move through the level as fast as possible. When I did this run I beat the level in 6 minutes and change.
Refer to "Super Boss Killer" for tips on beating the bosses.
