Got this in 5 minutes.
Pick Hard difficulty & then keep pressing/holding
until the timer starts. Once it starts, just let the timer run out. Once it runs out, hit "OK"/"SLEEP" until the achievement pops.

This achievement can be knocked over from the start.
- Select HARD at opening Menu.
- Once you go through initial Dialogue about your Daughter, continue clicking through until you have to Treat a "Rutger Hoof"
- Once you are treating them, on the top left hand corner will be a timer (on HARD it start s at 180 seconds).
- Let this timer run out; you will then be told the patient needs more treatment, click OK then Click SLEEP.
- You will then be advised you don't have enough Kapok (Currency) to pay maintenance
- Get Exiled and Achievement will POP
