I received this ending as part of a Structure Support playthrough, using the Easy difficulty. I'm not certain if the number of patients treated/personalities saved makes a difference here (other than cash on hand) but if anyone has any further info on situations that might invalidate this ending then let me know in the comments and I'll add the info to the solution.
The following decisions lead to this ending:
-Ignore the Moonrise fax and report it to the Structure
-Report the Disruptor you discover immediately
-Visited Boris the technician after the Disruptor fried my gear. I forget if this event was mandatory or not, but it's referenced at the point of erasure later so it must be significant
-When you're invited to scan the Constuctor (this was Day 29 for me), declare him Sane
-Shortly afterward (Day 36 for me), the Constructor invited me to become the Mind Sculptor, which I accepted
-Upon visiting your daughter, chose to erase her personality
-On Day 42, you'll be informed that the weapon is working perfectly and this ending will be the only one available
This playthrough can also set up the endings Not My Daughter and Catastrophe, if you simply rewind to the Mind Sculptor offer and choose "Comfort Her" when you meet your daughter.

Boris is definitely necessary for this. The option to erase her personality only comes up if you visited him.
FYI, it's a separate yellow ! event after turning in the disruptor - this is not for going to Boris to fix your gear (an option if you don't turn in the disruptor). I suggest to tweak the solution to make this clearer as I went to Boris to fix my gear and then was locked out of erasing my daughter.
