How to unlock the Inception achievement in Minecraft - Definitive Guide
Updated: April/20/2020. It has been a long time (Nearly 2 years?) since I haven't updated this solution. I might as well do so now. I am planning to update this solution with additional videos that hopefully will let you pop this rather glitchy achievement. I also will be clearing most of my old post below and renew the post.
One fair tip. Before getting started on the Xbox Achievement. When you get all the materials and items you need for the achievement. I would recommend Save & Exit and make a copy. Seeing as this achievement can be glitchy, If the copy world doesn't give you the achievement after a few minutes or so. Delete that Copy World and copy the main world again and do a different guide from one of the other videos below. Hopefully this tip helps! (To make a Copy World, Click on the Pencil looking icon before entering the world. Then from there, head right to the same page. Scroll or head down til you see the option to copy world.)
I got this achievement on October/19/2017 by following the Guide in a certain video. The process require you to slow down and repeat the steps of either the lever or redstone torch a few times. If a video doesn't work, Please do mention it on the comments.

Need one regular piston, one sticky piston, and one redstone torch.
Place the torch near the regular piston to activate it. Then, remove it and place near the sticky piston to activate it. When the sticky piston pulls back, it will pull the regular piston with it and that's when the cheevo pops. ".

Yes this method worked for me to. The achievement took about 30 seconds to unlock after doing it.

This achievement is only obtainable in Minecraft version 1.2.3 or later (stable and released to the public on October 18, 2017). If your game is not running the current version, you'll be prompted to update your game to newest version.
If you have played Minecraft: Xbox One Edition (Console Edition), read below on how to sync your old worlds over to this new version.
The basics (assuming you have it):
10 Redstone (approximately)
1 Lever
1 regular Piston
1 Sticky Piston
The achievement:
- Look at the ground and place the Sticky Piston.
- Walk to the left or right side of the Sticky Piston and turn around (180°).
- Look at the ground in front of the Sticky Piston (piston is to your left/right) and place the regular Piston.
- Both the Sticky Piston and regular Piston should be touching so that when either one expands, it will push the other.
- Place a trail of Redstone on the ground about 3 blocks away.
- Place the Lever at the end of the Redstone trail.
- Press , to turn on the switch.
- The Sticky Piston is powered and expands, pushing the regular Piston away.
- Break 1 piece of the Redstone trail that leads up to the Sticky Piston.
- The Sticky Piston now retracts because power is interrupted, keeping the regular Piston attached to it. (this is due to the sticky surface)
- Now make a new Redstone trail toward the regular Piston.
- The regular Piston is powered on and expands, pushing the Sticky Piston away.
Achievement unlocked!
Note: If the achievement doesn't unlock right away, wait about 1-5 minutes.
But what if you were starting a new world and you don't have the items pre-made listed above? Here is what you need:
4 Wood (tree trunks of any kind)
16 Cobblestone
1 Slimeball (or 1 Slime Block; can be crafted into 9 Slimeballs)
12 Redstone
5 Iron Ore (or 5 Iron Ingots)
Any type of fuel for smelting Iron Ore.
The process:
First, find a tree (yes, a tree) and punch it (mine it with your bare hand; ) to get a piece of Oak Wood. Or better, use any type of Axe if possible.
Repeat this process again with another 3 more pieces of Oak Wood.
Press to open the Crafting menu. Highlight "Oak Wood Planks" (or whatever type of planks for the type of wood you have) and press to make 16 Oak Wood Planks using the 4 Oak Wood we obtained earlier.
Now highlight "Crafting Table" and press to craft one. This will use 4 of the Oak Wood Planks we crafted earlier.
Now highlight "Stick" and press to craft 4 Sticks. Press again to craft 4 more Sticks. This will use 4 Oak Wood Planks we crafted earlier.
You should now have 8 Sticks.
Now highlight "Wooden Pickaxe" and press to craft one. This will use 3 Oak Wood Planks and 2 Sticks we crafted earlier.
Press to exit.
You might want to consider breaking the Crafting Table (mine with ) so that you can take it with you. We will be using it again later.
Find some smooth Stone (or Cobblestone) and mine it with the Wooden Pickaxe (or any other better Pickaxe) in hand using . We need at least 16 blocks of Cobblestone.
Look at the ground and place your Crafting Table down.
Press to open the Crafting Table.
Highlight "Lever" and press to craft. This will use 1 Cobblestone obtained earlier and 1 Stick we crafted earlier.
Highlight "Stone Pickaxe" and press to craft. This will use 3 Cobblestone obtained earlier and 2 Sticks we crafted earlier.
Highlight "Furnace" and press to craft. This will use 8 Cobblestone obtained earlier.
Press to exit.
You might want to consider breaking the Crafting Table again (mine with ) so that you can take it with you. We will be using it again later.
Here is the long and tricky part:
----- If you do not have 1 Slimeball (or 1 Slime Block), 5 Iron Ingots, and/or approximately 10 Redstone, continue reading this section. -----
You need to bring 2 Sticks, 1 Stone Pickaxe, 1 Furnace. You might consider bringing a Sword too if you encounter other hostile mobs.
You'll need to mine under ground, but never dig straight down.
Some of the things you are looking for are Slimes (a hostile mob; will not spawn in a world loaded on Peaceful difficulty), Coal Ore, Iron Ore, and Redstone Ore.
Kill a small Slime to obtain 0-2 Slimeballs.
Iron Ore can only be mined with a Stone Pickaxe or better (e.g. Diamond Pickaxe).
You'll need at least 5 total Iron Ore to make 5 Iron Ingots with a Furnace.
Once you have collected some Iron Ore and Coal...
Look at the ground and place your Furnace down.
Press to open the Furnace interface.
Highlight your fuel (we're assuming that you'll be using Coal) and press to quick move it to the fuel area (bottom).
Highlight your 5 Iron Ore and press to quick move it to the ingredient area (top).
As soon as both the fuel and ingredient are in the Furnace, a flame will be visible and the smelting process will begin.
The final product will made on the right. Once it is finished, highlight "Iron Ingot" and press to quick move it to you inventory.
Press to exit.
Now that you have your 5 ron Ingots, we'll be using 3 of them immediately.
Look at the ground and place your Crafting Table down.
Press to open the Crafting Table.
Highlight "Iron Pickaxe" and press to craft. This will use 3 Iron Ingots smelted earlier and 2 Sticks we crafted earlier.
Press to exit.
You might want to consider breaking the Crafting Table again (mine with ) so that you can take it with you. We will be using it again later.
Now go looking for and mine Redstone Ore.
Redstone Ore can only be mined with an Iron Pickaxe or better (e.g. Diamond Pickaxe).
The last part:
Once we have our Redstone, we can finish the process. We only need 2 Redstone now to make the Pistons, the rest will be used as Redstone wiring.
Look at the ground and place your Crafting Table down.
Press to open the Crafting Table.
Highlight "Piston" and press to craft using all materials we have. This will use 6 Oak Wood Planks we crafted earlier, 4 Cobblestone we obtained earlier, 2 Redstone we obtained earlier and 2 Iron Ingot we smelted earlier.
You should now have 2 regular Pistons in your inventory.
Highlight "Sticky Piston" and press to craft. This will use 1 regular Piston we crafted earlier and 1 Slimeball we obtained earlier.
You should now have 1 regular Piston and 1 Sticky Piston in your inventory.
Press to exit.
Now you have a regular Piston, Sticky Piston, Lever, and enough Redstone to continue as explained earlier in this guide.

Still on 1.2.2 for Xbox and following these steps does not allow me to update. Any idea whats wrong?

If you installed the 1.2.3.3 update while Minecraft was running, you probably need to quit Minecraft and load it up again.
Check the "Queue" section of My games & apps to verify that an update/DLC was installed for Minecraft recently. If you install other games/DLC, it might not show it.
When you load up Minecraft, you can verify that you are running version 1.2.3.3 by looking in the lower right corner of the title screen.
Also, you could try power cycling your Xbox One console if needed (which I've added to the solution).
I also have added a video to the solution above.
Check the "Queue" section of My games & apps to verify that an update/DLC was installed for Minecraft recently. If you install other games/DLC, it might not show it.
When you load up Minecraft, you can verify that you are running version 1.2.3.3 by looking in the lower right corner of the title screen.

For this achievement, you have to place a regular piston down then place a sticky piston in front of it so they are facing each other. On the side next to the regular piston place a lever down and pull it so the regular piston activates and pushes the stuck piston back. Press the lever again so the regular piston isn't pushed out. Now place a lever next to the sticky piston and pull it twice so the regular piston gets closer to the sticky piston.
