While in the End wilds or Broken citadel mission find a pile of glowing purple cubes that you can walk in. Ideally once where you are entirely covered. Stand in and use the totem of regeneration, soul healer, and or any other health regenerating artifacts to keep alive until the affliction "voided" x999 (times 999) appears then walk out and wait for the effect stack (number left of the icon and multiplier) of the affliction to wear off and it should pop.

If you have armour with Final Shout enchantment (tier 3) and a Totem of Regeneration you can just stand in the purple cubes and let your health dwindle, then FS activates your totem for you to regen faster than you're dying. You may need to use your health potion once (mine got close but didn't need it) just to be safe but if you've got that setup it's easy peasy.
My current armour already had it and I had a totem in my inventory, took about 2 minutes total 😎👍

Load up the Broken Citadel level on default difficulty. Near the start you should be able to find a Void Stack. Clear out any mobs around the area and then stand in the Void Stack. You will begin to gain the voided effect, which will slowly stack to 999. You will need to heal through the damage as you wait for the full stack. Once you reach voided x999, step out of the stack and wait for the effect to end (90 seconds). As long as you don't die during this process, you will get your achievement
