For anyone trying to get the items here is the full list of Gear Drops and Rewards. I had issues getting mine to pop but it did even without the green check on all locations. I had no ? items and it didn't work but it eventually popped when I got another Snow Armor. To keep the list shorter I'm only listing the gear on the Apocalypse list because you need that difficulty anyway but some of the gear can be found on lower difficulties. Everything is left to right in order from the game menu so you can tell what's missing

Creeper Woods

All gear drops and artifact rewards for all base game missions and all 6 dlcs must show the item and not a ?.
Gear drops are found from chests and mob drops in mission and artifacts are found at the end of the mission in the mission reward chest.

*Things to note*
-Some gear drops and artifacts only drop on adventure or apocalypse difficulties
-Items can be gifted from a friend and count as unlocking as it dosent show a ? after meaning you unlocked it (ie. i was gifted a tome of duplication and never earned it thru the mission but it counted as unlocking)
-If you find the gear drops or the artifacts in the shops that were a ? and buy it, it appears to also count as unlocking the item as it reveals the ? on the missions that item is in (ie I bought ocelot armor from the shop and then dingy jungle showed i had unlocked the ocelot armor)
-Its been confirmed by multiple comments that items exclusive to the daily missions are not needed for the 100%

The achievement tracker appears to move up at certain milestones. while his went from 82% to 90%, mine from from 90% to 100% while using the xbox achievement tracker.
