The lost conduit and its rightful place are found in the secret level, "Radiant Ravine". Refer to the solution under the achievement "Glow and Behold" to unlock the level. There are two secret pathways in the level, shown below. Both of these secret paths are easy to find and visible on the map, there is a small gap in between them and the main area.
The first one has the lost conduit and is located a little after the beginning. Stand on top of the small hill/rock and roll over to the cliff to the side, then follow the path to the conduit. Collect the conduit and carry it until you get to the next secret pathway. Whoever is carrying the conduit spawns inviable walkways, which is how you get back to the main area and eventually to the rightful place. See picture/video below for help.

The second one has the rightful place and is a little before the end. The area has lava coming off a mountain, go under the lava to the right and follow the path to the rightful place to get your achievement. See picture/video below for help.

If you are playing with more than one person, everybody in your game will receive the achievement, just make sure your near whoever is placing the conduit. Difficultly doesn’t matter. Thanks to GtechApocalypse for giving me some tips on finding the areas. Let me know how I can improve by leaving feedback in the comments.
