How to unlock the Maxed Out And Geared Up achievement in Minecraft Dungeons - Definitive Guide
You will start seeing items with 3 enchantment slots once you start playing in Adventure difficulty. As far as I can tell, Default difficulty does not spawn items of this kind. You can upgrade items by pressing Y on the item, pressing left/right to select the enchantment and then A to upgrade. Each upgrade requires enchantment points that you get for leveling up. Regular enchantment costs 1,2, and 3 respectively for a total of 6, while powerful enchantment costs 2,3 and 4 respectively for a total of 9 points to level up to 3.
What this means is that, you will need to level up 3 slots of 3 items to level 3. So, absolute minimum required enchantment points are 54, so you need to be at least level 55 to get this achievement. If powerful enchantments are included in the mix, you need more enchantment points. You can salvage items to get the points back, so do not be afraid to upgrade your equipments.
Edit: The enchantment in each of the slot may have to be unique and not same as the one in another slot. Thanks grrrrnade for this suggestion.

I can confirm that these also count toward your total and count as a level 3 enchantment. I just popped the achievement as soon as I started the Trial. I was one enchantment short prior to this. Not sure if this will be patched but for now it seems to work.
For reference I had 3 weapon enchantments maxed, 2 armor enchantments maxed, and 3 bow enchantments maxed. The Daily Trial gave me a temporary Looting enchantment (a weapon enchantment and one I did not have already) and the achievement popped as soon as I started the Trial.
I guess the interesting takeaway is that the achievement calculates 9 total enchantments and not necessarily 3+3+3 for the requirement. In my case with the added temporary weapon enchantment this meant I had 4+2+3=9 achievement popped.
Still no achievement. It says I'm only at 33% for this achievement.

The powerful enchantment costs more points so if you are purely only after the achievement make sure you upgrade the common ones.
Also, the enchantments must be different, if you select two of the same it will not register
Three enchantments, three levels, and three pieces of equipment.
Enchantment Lv1: +1 ep, Lv2: +2 ep, Lv3: +3 ep
6 enchantment points to cap out one enchantment
18 enchantment points for all three on one item
x 3 = 54 points
There might be some times where you'll end up needing to go with a Powerful enchantment though...

With that value range in mind, another suggestion: try not to worry about this early on until you're around Level 75 anyway. You can easily burn through levels once you've got access to the ??? secret level, and you'll also be doing a lot of grinding to try and get better equipment to play through Apocalypse anyway. Furthermore, you won't be able to unlock some gear with three enchant openings without playing on late Adventurer or Apocalypse difficulty anyway. Another option could be to simply get them gifted to you, though depending on your level, you might still have some challenge with having enough points to maximize everything.
You will need to acquire a melee weapon, a piece of armor, and a ranged weapon with all three large diamonds available for enchantment. Usually, this will mean progressing far enough into the game to start getting gear with power level 55+ to have all the available enchantments (but this does require RNG on your gear drops, so power levels can vary). Now, that you have all three gear pieces with all three large diamonds available, it's time to level up those enchantments. You need to level up a small diamond in all three large diamonds, for all three pieces of gear, to tier 3. Doing this for just one large diamond will unlock the Worked Like A Charm (20G) achievement.
It costs enchantment points to level up enchantments. If you decide to sell an item that you have enchanted you will get your spent enchantment points back. Enchantment points are earned through leveling up, so if you are level 50 you will have 50 enchantment points to use. Please see Expert Explorer (100G) for a XP farming method to quickly level up and earn enchantment points.

You will need to be at least level 55 to have enough enchantment points to complete all the leveling up that you need to do. Finally, make sure to equip all three fully leveled pieces of gear.
