Tutorial users can find the Notch apple already made at the following coordinates:
x: 318 y: 149 z: -188
Otherwise, players will need to create the Notch apple the normal way by collecting 72 gold ingots and an apple. The apple can be found in chests, through trading with villages, or by chopping down trees and waiting for the leaves to decay. The gold ingots can be collected through the mining and smelting of gold ore. Gold ingots can also be collected by killing Zombie Pigmen in the Nether, or by finding them in treasure chests scattered around the world.
Gold ingots can be crafted into Gold Blocks under the decoration tab.
The Golden apple can be crafted under the Food tab.
There are two golden apple recipes. The one involving gold blocks is known as the Notch apple. The recipe involving gold ingots is useful in curing Zombie villagers.

The other guides are excellent, but rather outdated because of the new tutorial. So I hope that this helps with the NEW TUTORIAL WORLD. (7-2-2017)
As stated elsewhere, you need 1 apple and 8 gold BLOCKS. Luckily, the gold blocks are more common than apples in the tutorial world.
You can either make an iron pickaxe, or pick one up in the Redstone area. If you go into the Redstone area, turn left and go down the stairs to the chest. There's a handy iron pickaxe for you there.
Now you're going to need the blocks. There's one near the Elytra, but the other 7 are going to need to come from the towers. Grab your pickaxe and climb the towers.
The hardest part for me seems to be the apple. Luckily, there's one right near the tutorial village. Across the pond, inside the mine cart track, is another little village. Go to the building with the lava in front. Inside is a chest with apples in it. The coordinates are:
X: -99
Y: 86
Z: -41
Then take your ingredients, make your Notch Apple (which still says "Golden Apple") and eat it.

For anybody needing to get this in the NEW tutorial world, as of February 7th, 2017, this is for you. What you need to do is go to the Elytra sign after walking into the tutorial area. Get the Elytra out and put it on. To the left is a tower with rings all over it. Climb to the very top and jump off the point, using your Elytra immediately after. Then, glide onto a ring with golden blocks(make sure you have a pickaxe) and mine the ring. Finally, use a crafting table and craft the Notched apple.
There are two golden apple's, you need to eat the one that needs 8 gold blocks.

This achievement can be easily unlocked in the tutorial world by simply breaking the gold blocks from the rings on the towers and using them to make an enchanted apple (or Notch apple).
