How to unlock the Cover me in debris achievement in Minecraft - Definitive Guide
To get this achievement you will need:
-16 gold ingots
-16 Ancient Debris blocks
-Full set of diamond armor
-Crafting Table
-Smithing Table
-Furnace
How to get Ancient Debris blocks:
They spawn in the lower parts of the nether now. I just vein mined like you would in the overworld to get ore. If you aren't finding much it might be worth finding a soul sand valley biome because they are more common there. Though it will still take a while to get all the ancient debris that you need. (You need to mine ancient debris with a diamond pickaxe)
I'm gonna assume you know how to get the other items listed because they are all from previous minecraft versions.
Then once you have 16 ancient debris, just smelt the ancient debris in a furnace to make them into scraps. Then just craft them into 4 nether ingots.
4 scraps + 4 gold ingots = 1 netherite ingot
Then you should be able to use the ingots to upgrade your diamond armor into netherite using the smithing table.

I’ve noticed that the ancient debris appears more frequently in the new biomes.

This achievement is really quite a grind, however there are a few tips that can make it easier.
So, what this achievement asks is for you to have a full set of netherite armour. All you need for this achievement is a full set of diamond armour and 4 netherite ingots.
Getting diamond armour is easy, however getting netherite is not. Here is a guide on how to get it.
1: To craft netherite, you need 4 gold ingots and 4 netherite scraps. Netherite scraps are obtained by finding ancient debris, of which only two blocks are found per chunk. Ancient debris is only found from below y22, however is most commom at y15 (according to Minecraft Wiki). The easiest way to clear out space in the chunk to locate this rare block it is to place a bed and press LT. This causes the bed to explode as attempting to sleep in the Nether results in the bed being blown up. Trust me, using beds makes this achievement a lot easier.
2: When you have obtained four ancient debris (using a diamond/netherite pickaxe) , smelt it in a furnace to get netherite scraps. Using four of these and four gold ingots can be crafted into a netherite ingot.
3: After repeating this process 4 times u should have four netherite ingots. Then, in a smithing table, place a piece of armour and then a netherite ingot in the adjacent square. Then, you will be given netherite armour.
4: Wear all netherite armour at the same time and then you will have the achievement.
[Disclamer: netherite tools are not needed, only armour :) ]
Edit: Thanks for ecinrun for pointing out you need a smithing table instead of a crafting table.
Edit2: I have finally done this achievement, i had 14 out of 16 required ancient debris and had ran out of beds, so i copied the world into creative and found the coordinates for the remaining 2 and used those. This does make things easier if you are not bothered about doing it fully legitimately, but if not then don't worry. :)

How do you mine the debris? Is iron pickaxe ok or do you need diamond?

The nether achievements are not unlocking for me but are unlocking for everyone else, I am so frustrated.

Obviously the easiest way is to use the achievement worlds people have built.
If you don't want to use an achievement world use the seed of your survival world to make a creative version of it. Make sure you have the location coordinates on in both worlds.
Go into the creative world, go to the nether of that world. Once in the nether dig down until you are at the 14th level (the middle number of the 3 numbers)
Once down there use either a bed or dynamite to start blowing up the nether. Ancient Debris can not blow up, so as you destroy the area with dynamite the ancient debris will stay standing. Once you find some write down the coordinates. Once you have found your 16 blocks you can go to the nether in your survival world and know exactly where to go.
Bring fire resistance potions on your survival world, you will never know when you will break a block and have lava come get you.
