I also couldn’t get this trophy. We delete the game, turn off the Internet, and install it again, then create a map and play but do not download game updates. Apparently there is some kind of bug in the new update that prevents you from receiving this trophy.
During the game, we wait for the food scale to drop to zero and eat a piece of rotten flesh.

At the moment (version 1.17) obtaining the trophy is again impossible by any means other than rolling back the game to the very first version

There is no need to bother with reinstallation, everything is banal and simple. Somehow we spend our hunger to 0 to take damage from hunger and eat rotten flesh. The trophy is yours
PSif you didn’t give a trophy, then eat until your hunger is completely restored

I managed to do it in the old version. I brought my hunger to zero and ate one flesh, and fell out quite quickly.

I managed to get this trophy on 1.18.2 (2.36).
I don’t know if this is a pattern or an accident, but I will describe the last steps I took before receiving it:
1) Died of hunger. This may not be necessary, but in my case, I simply did not have time to eat the rotten flesh;
2) Dug a deep hole and place a block above your head;
3) I actively jumped until I ran out of satiety and had 1hp left (half a heart);
4) Ate 1 rotten flesh.

Hunger means the hunger scale has dropped to 0, then you have to eat rotten flesh
