How to unlock the Breaking and Entering achievement in Minecraft: Story Mode - Definitive Guide

Grab the potions, the redstone dust, the TNT and the button. No matter which potion you chose, you will have to follow some QTE. After that, QTE again, you will need to climb the tower and pour the water. Do the same with the two other towers. Follow the QTE to try to rip the heart off. Climb over your friend, and rip it off. The chapter will end after the next conversation.

Once you gain access to PAMA's core you'll have to face one of the more difficult enemies before you can deactivate PAMA. Once you defeat PAMA, save your friend and leave the core the achievement will unlock.
