After you've entered the portal at the end of Chapter 5, you will be teleported to a new world. First you will need to investigate the book right in front of you. Examine it with 
and a fight against zombies will starts. Follow the QTE instructions and you will escape the herd while the opening credits rolls up. After that, you will need to enter the mansion, follow the QTE to escape zombies again, and then look at the window above you. Smash it 
to enter, the chapter will end here, giving you the trophy.

You'll find yourself running through swarms of zombies. You'll see a mansion and you make you way toward the mansion. As soon as you get in the mansion the achievement will unlock.
