How to unlock the Camouflage achievement in Minecraft: Xbox 360 Edition - Definitive Guide
Camouflage - 30G (Requires Title Update 48)

While playing the new Elytra Tutorial, you can find a Skeleton, Creeper, and Zombie skull mounted on the wall at X: -143, Y: 108, Z: 74 (in a tree house) Just grab the one you feel most comfortable fighting and equip it as you would a helmet. Now make sure you're playing on Easy or higher (Not Peaceful!) and defeat that same enemy while wearing it's head. For example: Wear a Zombie Head and kill a Zombie.
No need to waste time going to the Nether and look for a Fortress.
IMPORTANT NOTE: Thanks to LordFightALot a solution has been found for anyone that was trapped with relocked Achievements on the day of Title Update 48's release! Do as follows and there is a 99% of it working!
"This is exactly what I did:
1. Start up the Xbox (it auto signed in for me)
2. Manually sign out of your profile on the Xbox dashboard.
3. Launch Minecraft.
4. Press Play Game
5. A popup appears and asks you to sign in. Press Yes [I want to sign in].
6. The achievements popped right away on the world selection screen."

You can also get Creeper (And Wither) skulls from the large banner-tutorial chest in the central town if you're not up for travelling.

In the new aquatic update, you can find a creeper head in a chest by the banner tutorial area. The coordinates are X: 46, Y: 82, Z: -110
After you get it, simply put it on and wait until nighttime and then kill a creeper while wearing it. There don't seem to be any other mob heads except creeper and wither heads, so your best bet is to kill a creeper. You should craft a diamond sword in order to make this easier. You can find an abundance of Diamonds after you exit the area where the beginners tutorial is. It's on the giant pickaxe in the sky. Simply mine it down and convert it to regular diamonds and you are good to go.
After you get it, simply put it on and wait until nighttime and then kill a creeper while wearing it. There are two ways to go about this. You can either travel a short ways to get four different mob heads, or travel almost no distance at all and get five creeper heads.
Underneath the giant elytra wings in the sky there should be a chest underneath them containing the creeper heads. There's also one in a chest above the slimeblock area.
If you want the other heads, you can travel upwards into a treehouse and find them in itemframes at these coordinates: X: -143, Y: 108, Z: 74
(It's right to the left of the big "Minecraft" sign as a reference)
