I did this on the first pitch with Jose Altuve vs. DeGrom. Make sure that you use X for a power swing. He always starts with a 100mph fast ball, so restart the game if you don’t hit it out.

Given that Jacob DeGrom is hurt at the time of writing this and not on live rosters, another option that I got this easily with is to do an exhibition against the Reds with Hunter Greene as their pitcher. Got the achievement on the very first pitch against him. Guy throws 100mph IRL a lot and seemingly makes a good option for this achievement.

Not sure if any of this helps someone but: I used the Boomers against the Reds, and had maxed human contact, power, timing, solid hits, as well as cpu pitcher control and consistency.
Also of note: You only need 100 mph, but it has to go out of the park. In-park home runs wont count for this one.

Seems like the AI throws balls more consistently over 100mph in retro mode. With some trial and error this shouldn't take more than 15-30 min. I played boomers vs boomers

From the main menu, go to Create and Edit Player. Pick any team and edit their best starting pitcher, go to Attributes, Pitching, and find their 4-Seam Fastball and push the velocity up to max. If they don't have a 4-Seam Fastball, either pick a different #1 starting pitcher to edit, or go to Pitch Types for the player and give them a 4-Seam Fastball and then go in and max out the velocity on it.
Then put hitting difficulty on Beginner and play a game against the team with the pitcher you just edited. Make them the home team and you the away team. It can also be helpful to rearrange your lineup to put your best power hitters at the top. You can also edit sliders in the pause menu (Settings > Sliders) to set max Human Contact, Power, and Solid Hits.
Then just swing for power () on every pitch and fairly soon you should hit a home run on a 100mph pitch. If you don't get it in the first inning, Pause -> Exit -> Confirm Restart and try again until you get one. Using this method I got it on my 3rd batter of the game.

You'll want to play against a pitcher with a 99 rating on his fastball. I did it against the Mets with DeGrom as the opposing pitcher. Make sure you are away so you bat first. With the batting difficulty on beginner you should have no problem hitting a homerun. If you hit one off of a 100 MPH pitch the achievement will unlock.
