Your team's free agency signing period is ranked on 3 concepts: Pitching, Batting, and Fielding. By getting top players AND (very importantly) improving your existing positional players for all categories you should make 5 stars.
I selected the Pittsburgh Pirates so that all 3 March to October achievements could be unlocked with one playthrough, and I played on beginner difficulty so that I could easily make the playoffs, and win the World Series (hoping this increased my likelihood of drawing top ranked free agents)
With an Offseason Budget of $132 million, here's how I recommend playing it to get a 5 star rating. Fill positions 1, 2, and 3 with the top ranked free agents. After selecting 1, 2, and 3, you have to wait a week before you can do anything but sim day by day because pop-ups will occur-- for example Shohei Otani immediately got signed but I was able to offer a last chance contract (he did not accept). Interested free agents will occasionally reach out to you and always make it max year and adjust money so they reach 70% interest.
Once targeted top-ranked free agents reached 50% interest, offer them a max contract and cross your fingers. I ended signing about 11 players until my budget ran out in week 8. Here is the list of who worked out for me in order for first to last. This ended up being a 5 star offseason and the achievement popped.

I used "Go offline" to give myself endless attempts. I first won the World Series as that seems to help based on the comments on this thread. I then got to the first week of the big board and then went to Xbox Settings > Network Settings > Go Offline. Then I would go through the free agency period (I typically signed 16-18 players) and focused on Ratings on the Depth Chart screen. Once you are ready to see your rating, go to Advance to Offseason. If its less than 5 stars, then just Manage, delete Console save, go online and restart from the week 1 big board. If its 5 star, great! Good job! Now quit the game, go online and load it back up. It should load right to the big board before you “Advance to Offseason”. Do it again and Pop Pop!
Original Solution - IMPORTANT- super easy to miss this achievement by getting 4, 4.5 stars and there is NO way to back up your save. My new theory is trading most of your starters prior to trade deadline to take on as many low rated players / minor leaguers will help somewhat. Just know it's possible to royally screw this up. Won the World Series with the Tigers and in the off-season only got 3 and a half stars with a $150M+ budget. Tried to sign the top free agents offering max contracts with max salary but missed out on a fair few, only improved SP to 3rd, RP to 10th, 3B to 6th, LF to 9th and 17th overall. Signed Nola, Hader, Chapman, Happ, Myers, Kershaw, Cron, Floro, Frazier, Kiermaier, Wong and Boxberger.
Not sure what criteria the game uses to determine the star rating. Purely down to positional improvement? Overall improvement? Or does being reckless with the budget offering max salaries for max duration affect it too?
UPDATE: I was able to achieve this with the Boston Red Sox after winning the World Series. Signed Ohtani, Urias, Happ, Hicks, Rosario and Floro.
I've also seen this achieved with the Cleveland Guardians by signing Ohtani, Urias, Rosario, Cron, and Happ.

found for this achievement I had some trouble unlocking it after 3 try’s with the dodgers once I switched to the Red Sox and finally won the World Series with them I signed the highest rated players that my team needed( I think this part is crucial, signing players that fit your team) so if you use a team with lots of glaring weaknesses it should be easier. The highest player I signed was a 90 overall and 3-5 low 80’s but they significantly improved my team. I expected to not get it but boom 5 stars, I believe resigning important role players might have helped too (Devers and Sale were resigned)
