How to unlock the Old School achievement in MLB The Show 23 - Definitive Guide
This achievement is fairly straightforward if you're familiar with the save rule, although there is a wrinkle that I'll get to momentarily.
For those unfamiliar, a pitcher is credited with a save in the following circumstances (per MLB's official rules):
1) He is the finishing pitcher in a game won by his team;
2) He is not the winning pitcher;
3) He is credited with at least ⅓ of an inning pitched; and
4) He satisfies one of the following conditions:
a) He enters the game with a lead of no more than three runs and pitches for at least one inning
b) He enters the game with the potential tying run either on base, at bat or on deck
c) He pitches for at least three innings.
If you're in the habit of winning blowouts, the three-inning save (4c above) works regardless of the score; that's how I got the achievement. Simply put in your closer to start the 7th inning and have him finish out the game. If your lead is 3 or fewer runs, you can wait until the 8th, but make sure there are no outs in the 8th when they come in. You cannot surrender the lead at any point after the closer enters - even if you eventually win the game, the save situation has been lost (assuming he finishes the game, he'll be credited with a win and a blown save).
The aforementioned wrinkle: "Closer" appears to be strictly defined by the game, and it appears that it's not only the "closing pitcher" slot in your bullpen but also a pitcher whose position is designated as "Closing Pitcher". I recorded a two-inning save in one of my first Diamond Dynasty games with whatever scrub the game started in that spot on my roster and didn't get the achievement, but I was successful on my next try after acquiring Ryan Pressly (who's listed on his player card as a "Closing Pitcher" rather than "Relief Pitcher") from the Captains choice pack in the Season 1 XP Program and inserting him into the CP spot in my bullpen.
If you are playing a game of fewer than nine innings, adjust the entry point of the closer accordingly. I believe a two-inning save is possible even in a three-inning game, as the starter can be credited with a win for a single inning pitched; of course, in that case you'll need to have a lead of three or fewer runs after the first inning.

Just brought in Josh Donaldson with a 1-run lead starting in the 2nd inning. Pitched the 2nd and 3rd with him and was credited with a save but no achievement. So I don't think 3 inning games count for this.

I'm pretty sure Donaldson is not designated as a "closing pitcher" (the "wrinkle" I mentioned in the solution). I don't think any of the NLB players have that designation, so you might have to use someone else regardless of the number of innings.

Just brought in Josh Donaldson with a 1-run lead starting in the 2nd inning. Pitched the 2nd and 3rd with him and was credited with a save but no achievement. So I don't think 3 inning games count for this.

I used a solution that is easier than all 3 of the existing guides. It can be done in a 3 inning exhibition game.
Steps:
Steps:
- Choose Exhibition game from the home screen
- Select any double A team for the Away team. Like the Flying Squirrels
- Select an all star team for the Home team. I used AL ALL-STARS. Play as the Home team.
- On the Options Page change Innings to 3
- (optional) On the Options Page turn Quick Counts On. This will make it go a little faster.
- Select your starting Pitcher
- Select Advance on the Batting Order page
- Pitch the top of the 1st inning with your starter. Hopefully you don't give up any runs.
- In the bottom of the 1st inning you must score when it is your turn to bat. If you do not score a run you will need to restart the game. Your team must have a lead when you bring in your closer in the 2nd inning.
- In the top of the 2nd inning, bring in your closing pitcher. It is the pitcher at the very bottom of the bullpen list with CP as their position.
- Pitch 2 innings with the same pitcher without surrendering the lead. This should not be hard if you are playing a double A team. You can probably through the ball straight down the middle.
- Achievement pops at the end of the game!

I did this in Diamond Dynasty.
First off, go into “Manage Squad”
Scroll Down to “Bullpen.” In this section there will be 8 pitchers. The bottom right one will be labeled “Closer”
Here’s the catch. In order for this achievement to trigger, you MUST have a pitcher whose position is CP (closing pitcher) in the CLOSER spot. If your CP is ANYWHERE ELSE in the bullpen you will not get this achievement. I had my CP in the LRP position for so long wondering why the achievement wouldn’t pop.
Next load up a Play vs. CPU match in Single Player Modes on Rookie Difficulty. DO NOT GET A LEAD LARGER THAN 3 RUNS. If you lead by 4 or more it is not considered a “save”. In the 7th inning, pause the game, go to the Manager tab, and click on Bullpen. Warm up your Closing Pitcher by selecting them and choosing “Warm up”. I believe they will be at the bottom of your list.
Play the 7th inning. Once you get to the 8th inning, once again pause the game, go to bullpen, scroll down to your pitcher, and select “Put in Game” Finish the game and win.
NOTE: Closing Pitcher must enter when you are ahead by 3 runs or less. If the computer comes back to TIE or TAKE THE LEAD it will not count as a save. You must hold on to the lead.

For the Red Sox, the game lists John Schrieber as the CP and Kenley Jansen as an SU, even though Kenley Jansen is the closing pitcher in real life. I kept trying to get this achievement as John Schrieber, but it never unlocked.
Before going for this achievement, Google who the real life closing pitcher is for a team. When I used Kenley Jansen as the closing pitcher in the last two innings, this achievement finally unlocked for me, even though he is listed as an SU in the game.
