Welcome Back to another year of The Show!
Pretty easy to set this up, executing it can be inconsistent to say the least.
Note:
If you have 2 consoles or a second account with Gamepass Ultimate using cloud, you can boost this and other achievements with yourself using online Exhibition - Credit to NoHeroes94 for confirmation on that!
Set difficulty to beginner.
Go into Sliders:
Set all CPU hitting sliders to zero
All User Pitching Sliders to MAX
My foul consistency for the CPU was MAX as well but not sure that matters
Go into a game and make sure your team is the one on the right (Pitches first)
I went All Stars v Birmingham in the AAA division.
Once on the mound, Intentionally walk the first three batters ( + ) and the bases will be loaded
Now you need to strikeout the next 3 pitchers, they can foul balls but just cant register any hit. I use classic pitching and found fastballs on the edges of the box to be more consistent in getting them to swing and miss or foul but if your confident in pitching just use what you would normally to strikeout players. My strategy is more of a beginner friendly one, I used this method and I took around 4 attempts and I play this every year!
NOTE:
If you have struck out one player and mess up you can always go for a double play on the next at bat to end the inning and that will net you the A Double Play Would Be Nice Here achievement!

Good guide.
I made two attempts without changing the CPU's Foul Consistency, and they ended up getting a hit at one point. So I backed out, changed it to Max, chose Gerrit Cole with the Yankees as he has a Knuckle Curve which was a good 2-strike pitch. Then I got it first try.

Can you get achievements with two controllers? It’s not a listed exclusion but one unlocked no achievements with two controllers.

You can unlock this achievement through a bases loaded no outs scenario in the Storylines mode, specifically the eighth and final episode (1924 World Series Game #10) chronicling the career of Negro Leagues star pitcher and shortstop Jose Mendez. (He can be found in The Negro Leagues Season 2 category on the Storylines selection screen). You'll be put in at the top of the 9th inning, with a 5-0 lead and of course the bases loaded for the opposing Hilldale team, and given the conditions needed to unlock the achievement you essentially have to strike out the side. If you ever mess up (walk, hit batter, and of course any batter being able to hit a pitch that puts the ball in play that ends in either a base hit or putout), simply pause the game, move to Exit in the pause menu, and then select Confirm Restart.
It's well recommended you try out all the pitching interfaces in the Options Explorer menu that works most comfortable for you in getting the best pitches in both accurate location and consistency and boost the chances of striking out batters consistently. I've gotten comfortable using the pulse interface, which was what I went with when starting out on this game. I'm putting this solution in as simply an alternative that offers the convenience of not needing a second player to help co-op with you in any game you setup with regard to having control of the opposing batters.

This can be done in the tutorial of Diamond Dynasty, after customising your squad for the first time there will be a challenge to win a game at the top of the ninth, complete this and the achievement will pop.

I did this in the tutorial and the achievement didn't unlock.

My tutorial was for hitting a walk-off and that did pop the achievement. I didn't see a pitching tutorial for DD
