Apparently this is missable, but I'm not sure why yet as I've not started the game. It's probably worth adding the information to the solution though. There are a couple of notes on this page:

However, the formatting is all messed up for me no matter which browser I use.

I'm pretty sure this is missable, because you can't revisit the section with heart number 4 after collecting the fragtment and leaving this area. You have to collect the heart and reveal the map BEFORE you leave the portal/tear. Otherwise the tear disappear and you can't go back. In that case you miss the map AND heart achievement. shock
