This will most likely unlock through normal gameplay but if it doesn't you can easily do this by playing through the game and then returning to the starting area.
This will mean you're able to beat the monsters quickly and without taking damage resulting in a likely 5 star win.

You can get this achievement and the You'll Never See It Coming achievement at the same time. you need to run into a group of wild monsters anywhere in the map and kill them in one turn. It's not very hard to do, once you level up your monsters to a higher level you can back track and easily kill low level monsters. You could also use counter elements to beat monsters.

At the end of winning every battle you will have a Combat Result menu pop up. As a rule of thumb, the higher score that you get, the better loot that you find. As you can see in the image below, your score is determined by how much health you have, how many combos you did, how fast you finished the battle and how many buff/debuffs you used.
