I got this achievement unexpectedly in a random battle against some lower level enemies.
I used Yowies Glorycast Lvl 3, Goblin Kings Earthquake Lvl 2 and Frostys Ice Storm Lvl 3. Glorycast raised the combo meter to 7, Earthquake raised it to 15 and Ice Storm raised it to 26 for the achievement. Some skills that helped raise the combo meter include Magic Proc for Frosty which triggers an additional hit for every attack, and Combo Buffing for Yowie which gives a random buff every time a buff is applied.
The skills i used all require the monsters to be at level 30, just to give you an idea of when to expect this achievement to be attainable. It might be possible to get this achievement before then but i kind of doubt it, as the Lvl 30 skills all add additional hits/actions to the skills. Also, there will be a ton of similar skills to the ones I used that will work just as fine for raising the combo meter to 25x, just experiment with them until you find one that works if you don't want to do it exactly like I did.

For this trophy you will need to achieve a combo of at least 25. This will likely not be possible until later in the game when you level up your skill points and attack effectiveness.
Each attack will have a description of how many times the attack has the potential to work. In the below image you can see the Light shifted Troll's Poison Eater attack has 5 bites, this does not always mean it will land 5 times as attacks can and do "miss". Furthermore you can see that this attack deals an additional 30% hit for every poison effect on the target. The Champion monster in the image has 7 stacks of posion which you can see above the health bar, this was performed by the Fungi monster in the party before the Troll. This means that the aftereffect of the Poison Eater attack has potential to hit a further 7 times, thus increasing this monsters attack combo to a potential 12 hits.
There are near endless ways that you can achieve a 25 combo with the 101 monsters in the game. Pay attention to what each attack does and see if they work hand in hand with another monster that you can take advantage of and enhance the combo meter to unlock the trophy.
