How to unlock the Skull Kabob achievement in Mortal Kombat 11 - Definitive Guide
To unlock this achievement, you will need to get a fatality against the same opponent 25 times in any of the Tower related modes; including Towers of Time or Klassic.
My recommendation is to follow these steps:
- In the main game settings, change Rounds to Win to 1. This will allow you to get a fatality in every fight.
- Go to the Klassic Tower mode, and choose Survivor (only tower where 1 round works).
- At the character screen, select any character you're comfortable playing as, and set the difficulty to Very Easy.
- Perform a fatality in this fight, by getting the opponent to the Finish Him state, and then doing the button prompt. If you're not certain what the prompts are, go to the Move List and pin it to your screen.
- Proceed to the second fight, and as soon as you gain control of your character, pause, go to Tower Select and Confirm, to start the tower again.
- Repeat this process until you perform the fatality against the first opponent of the tower until you have 25 (or more), and unlock their severed head.
- Go to the Krypt, go to the Warrior Shrine (connected to Courtyard), and place the head on the respective spike. Unlock the achievement, and gain the rewards of the chest.
Additional Notes:
- It is possible to do this using an AI fighter if you prefer.
- Leaving and returning to the Klassic Tower mode will shuffle the towers, so you may need to do this a few times to target the fighter you want if done across multiple session.
- Many users have reported that Brutalities also work, but they seem much less consistent.
- You will not get the notification of the head reward if you haven't been in the Krypt yet, however it will be your inventory once you do, if you've done the 25 fatalities.
NOTE: As of a recent patch, heads now only require 25 fatalities.

Following these steps, a head can be earned in as little as 35-40 minutes. Since the character chests at the warrior shrine can also unlock victory and intro poses, this may be a decent grinding method for those interested.

In order to impale a head on the Warrior Shrine, you must perform several Fatalities on that specific character a number of times in Towers
This is one of the more time consuming trophies in the game, because you will need to utilize a specific number of fatalities against a certain character in tower battles (Klassic or Towers of Time), in this case, you will need to do around 25 to 50 fatalities against the character you want to do this trophy with.
For example, we're going to use Erron Black, whose spot in the warrior shrine is the third one on the right side (coordinates: -1096, -5475). Afterwards, you will then need to go to the towers (can be done in both the Klassic Towers and in the Towers of Time) and you will need to have him as your first opponent in the Tower you wish to do (the Survivor Tower recommended if you're playing in the Klassic Towers since all matches end in one round), as seen in the screenshot below:

In the event where he isn't the first opponent you fight in the tower, be sure to back out and go back until you see him as the first opponent. Afterwards, you will need to fight him and execute a fatality against him when you deplete his health in the first or second round (depending on the tower you are playing in). Once he is defeated with a fatality, you will then need to quit that tower and start again with the same character or a different one and use a fatality against him for the second time. Repeat this process until you have acquired his severed head:
Now that you have acquired it, you will then need to head back to the Krypt and place his head on his spot in the warrior shrine. Where you will unlock some extra items including a victory scene, brutalities, skins, and much more. It should be noted that any victories you have unlocked will count towards the Victorious trophy. In addition, if you have the severed heads for all of the base game's characters (including Shao Kahn), you will be able to unlock Shang Tsung's throne room, where you will unlock Intro sequences that can be used towards Have We Met.
Impaling a head requires you to get 25 fatalities on a specific character, and have progressed to the entrance to Goro's Lair in the Krypt. I won't explain how to get there, because it's pretty straightfoward, and the game kinda holds your hand.
The easiest way to boost the 25 fatalities is to go to klassic towers and scroll over to the endless tower. Make sure the first fight is a non-DLC character, as the DLC characters don't drop heads. Any character other than Shang Tsung, Nightwolf, Joker, Spawn, Robocop, Sheeva, Rambo, Terminator, Sindel, Mileena, Fujin, or Rain should work just fine.
Enter the tower with the intended first fighter. Beat them and do a fatality. Afterwards, load in the second fight. After it starts, pause and return to tower select. You should be able to repeat the tower with the same first fighter. Do this 25 times and you'll get the head in a notification after the fight.
Once you have the head, go to the entrence to Goro's Lair in the Krypt, and approach the spike that has the bust of the head of the character you just killed 25 times, and place it down. You should get the trophy, and a bunch of gear for the character.

In the Krypt, there is a place with pedestals of the heads of the game characters (there will be a screenshot in the additional tips).
We must impale the character's head on a stake. To get it, you need:
Perform 25 Fatalities on one character in any Towers. (the easiest way in the Classic Tower is Survivor, there is only 1 round.)
To combine with the Victorious trophy we do the following:
We put an AI fighter and let him hit 5 opponents in the Tower, in 99% of cases the AI does Fatality.
We only need to restart the tower on the 6th enemy. Pause and press - Select a tower, so the order of the opponents is maintained.

Guys, I confirm that ONLY Fatalities count!!! Don't repeat my mistakes, don't do brutality! I boosted the classic towers for 5 goals at once, and counted everything. As a result, I can definitely say that brutality does not count towards the score, I lost about 2 hours because of this. So I started everything with AI in 4-5 hours.

Among other things, they give Setrion a key for her head. There are three in total
Another one (the Kronika key) can be obtained by donating a little gold in the altar (I got it when I paid 50k at a time)
The last key is Shinnok's amulet and must be crafted. All ingredients except the last one drop out on their own, to get the last one you need to open a flying crap for 10k souls in the forest
Not far from the forge on a wooden bridge there is a closed door that opens with the help of the MK medallion (I don’t remember the exact name)
In this room at the very bottom and you can use these keys to get into a closed location
