How to unlock the All the Pieces achievement in Mortal Kombat X - Definitive Guide
Go into Extras, Offline Kombat History and then change the background, icon and border. Press B twice to back out and you should get a save prompt. Press A to save changes (you will get the achievements for individual categories if you haven't already).
Now go back in and set everything back to the original three selections again (Welcome, Mortal Kombat X and It Has Begun) and save. The achievement should unlock.

Once you unlock a Background, Icon, and Border with the exact same thing. Equip all 3 and save your badge. Then the trophy will unlock. Please note it has to be one of the named sets - the Recruit set will not work.
To unlock this achievement you need to enter your Kombat Kard, this can be found in Extras > Offline Kombat History. Once you have your Kard, open you need to hit to edit it. Then hit to change your background, here you should see a background that was unlocked when you choose your current faction. Press to show the details of the background on the right hand side and at the bottom it will say ' Equip Set'. Do this and it will change all three pieces of your Kard at once. Now press to exit out and when prompted save the changes to unlock the achievement along with "Back It Up", "Real Icon" and "So Bored."
For this trophy, you need to equip the 3 starting things that are on the top of the list.
Here are the names of each one:

- Background: Welcome
- Icon: Mortal Kombat X
- Border: It Has Begun
After selecting all these three cards, press twice, save the changes and the trophy will pop.
If for some reason the trophy doesn't pop, here is an alternate method:
While editing your Kombat Kard, you can press to view or hide info about that specific item. Once you are viewing it, many of these items can show the option Equip Set (right corner in the screen). Press to equip the entire set, then press twice to save the changes and the trophy should pop.

Everything is outrageously simple, you need to put a frame. background and icon of one fighter. For example, like mine, Background Scorpio, frame Scorpio, backdrop Scorpio (ATTENTION!!!!! there shouldn’t be a cartoon man on the backboard, put anything except him.)

And there must be stars... the background without a star was not right... I put all the stars at once

It is not necessary to put a background, icon and frame for some character, you can put the usual ones for a victory (there should be 3 victories in total)
