In order to unlock this achievement, the first thing you need to do is load up a Klassic Tower.
This can be found under ONE PLAYER --> TOWERS --> TRADITIONAL TOWERS
Once there select the KLASSIC TOWER and choose LEATHERFACE as your fighter. You then need to pick the KILLER variation. This gives you access to the BERSERKER STANCE.
YOU MAY CHOOSE WHATEVER DIFFICULTY YOU LIKE AND ARE COMFORTABLE WITH
To perform the BERSERKER STANCE, you need to input DOWN, BACK, 3 (A). Once this is done, LEATHERFACE will pull out his chainsaw and take a special stance. Once in this stance you have four options.
PRESS A (Again) - To Exit the Stance
PRESS X - Psycho Rush (A quick sprint into the opponent with the chainsaw)
PRESS Y - No Escape (A series of large swings with the chainsaw)
PRESS B - Running Frenzy (A running move while swinging the chainsaw overhead)
In order to unlock this achievement you need to perform all four of these moves in a single match (again, while in the KLASSIC TOWER).
Note: Each of these moves can be ENHANCED, but it is not needed to complete the achievement.
After performing each of the moves, the achievement should pop. Pretty easy.

For me, the achievement didn't pop until AFTER my matches were over......still +1 from anyways!!!

So go through the menus: One Player; Towers; Traditional Towers. Then select Klassic Tower (any diffikulty, although Very Easy would be easier, obviously)
For your kharakter, you'll need to be the 'Killer' version of Leatherface, this is the only one with access to the Berserker Stance.
When the match starts, you need to get into the Berserker Stance by using
, this also makes Leatherface bring out his trusty chain saw.
There are four moves you need to do from this stance in total, each using a different button - once in the stance, pressing again exits the stance (yes, this is necessary);

This trophy is awarded by hitting an opponent with every attack launched from Leatherface's Berserker Stance during a Klassic Tower match and then winning. This can be done only in Killer Variation, and is a set of three attacks that you have to hit. To enter Berserker Stance you input Down, Away, Cross (

facing right), and then input

to launch an attack ( again drops you out of Berserker Stance). This doesn't need to be all in one round but you need to successfully hit your opponent with all three attacks. You can launch the three attacks basically as soon as you enter the Berserker Stance input and Leatherface revs his chainsaw in front of him. If you hit all three attacks successfully the trophy will be awarded on the end of match screen.
