So I tried to use the in-game manual tracking from the settlement encyclopedia to help. Aside from slightly lagging my game when tracking everything at the same time, some settlements also automatically get un-tracked by the game. For sure it happens when a town changes hands, and I've seen it happen when after a village gets raided as well. Not sure about other triggers. Anyway, you can try this if you can eradicate all fighting in your game, like maybe in a save file where you got the Supreme Ruler achievement or executed every noble. I gave up and am now writing everything down instead.

Pretty sure a recent update must have busted this achievement or something. I've gone through the encyclopedia, made a list of every village, castle, and town. As I was playing I kept track of each settlement I visited. After reaching around 50% from natural play I went around visiting the remaining to just to get this achievement done. I didn't mark any settlement as visited until I reached the settlement. I would buy one item from every village and visit the keep of every castle and town, basically just try to interact with the settlement in some way. Have done this twice on two separate playthroughs and no achievement. Even went through the encyclopedia afterwards and started looking through each settlement to see if the notables there were still hidden. Didn't finish because that was giving me a headache, but for now no longer care.
