just complete the main co-op levels and unlock and complete the missing ones which can be found by finding the secret exits on the levels which blink and have a ? behind them on the world map.
There's loads of Youtube videos for most of the secret exits except for the one on 3-15. Luckily I found it and here's the video for it if anyone's still after it.

Apparently there's not a single video of this game of co-op level 3-12 on the entire internet, can't figure out how to get past the first screen of this level. OK, managed to figure it out, the timing is just really tight, so it didn't seem like the solution.

This can be done in any combination of Online/Local/2G1C clears. Meaning you can do some levels online, some by yourself in 2G1C and some Local.
