How to unlock the Carnage achievement in MURDERED SOUL SUSPECT - Definitive Guide
One of the many side missions you can play in Salem. You will come across Brad near the Police Station (left of Police Station if you are leaving it) and once you talk to him this question will be asked.
Was I the one driving?
EDIT: I put video link instead of writing as this will make more sense.
Credit goes to HarryNinetyFour. Old video was deleted so I posted a new one.

*MISSABLE*
This achievement can be unlocked either before you enter or after you exit the Police Station. This is a short side investigation unrelated to the main story where you help someone find peace and pass on to the afterlife. To initiate it, facing the Police Station entrance, walk to the left and around the ghost wall to the right. You will find Brad standing outside of his car which has crashed up on the curb. Speak to him and he will reveal that the night with his friends resulted in him crashing his car and killing his two friends. You need to search the area for clues and piece together what really happened.
- “B-RAD” license plate
- Scotch Whiskey
- SOC Gas Station Receipt
- Dented Trash Can (right outside of Police Station)
- Pieces of Tail Light (next to Dented Trash Can)
- Laura Pumping Gas (at SOC Gas Station)
- Rick Puking
- Brad Making a Call (at phone booth)
- Press and piece together the clues ‘Laura Pumping Gas’, ‘Rick Puking’ and ‘Brad Making a Call’ to reveal the ‘Mysterious Bang’ clue.
- Man next to Brad by door (possess him and select the ‘Mysterious Bang’ clue)
When you’ve found all the clues, return to Brad and speak with him. You will tell him what you found out and that he wasn’t the one driving. When the conversation is over, the achievement will unlock.

You'll find Brad on the way to the police station leaning up against a ghost car that has driven up and into a planter bed. Speak with him to begin the investigation into what happened.

Near the gas station and police station you will find a ghost crash victim. He believes that he killed his child and wife. Walk around and find the following clues: whiskey, license plate, gas station receipt. Having collected this evidence, head to the gas station. At the gas station we are looking for the following evidence: a vomiting person; Laura pouring gas; Brad makes a phone call; dented trash can; taillight fragment. Having collected evidence, we restore the scene of the incident: Brad calls; Laura pours gas; vomiting man. A loud bang startled them all. But what was it? We move into the man repairing the door and force him to remember with the help of the “Fatal Slam” evidence. It wasn't Brad who was driving, it was Laura. We tell him these joyful things and he goes to heaven with a calm soul.
