Unlocked in the "Ashland Hills Cemetery", you'll need to collect 13 heirlooms to unlock this Ghost Story and achievement.

The achievemen may be glitched. I have compleated 100% but achievement wont pop up.

Half of the ghost story achievements are glitched for me.

Yeah same as TotalKaosX1 the achievement wont unlock for me after getting 100% items.

This trophy is titled "The Heirloom" and it requires you to find 13 Old Heirlooms scattered throughout the Ashland Hills Cemetery.

Heirloom 1 - The first Old Heirloom will be found several minutes into your time in the Ashland Hills Cemetery. After getting passed the three demons, move beyond the original cemetery walls and head to the left then proceed up the hill.
A small touring group and a ghost butcher will be standing to the left of a stone gazebo mausoleum. The first heirloom will be sitting on top of the burial vault (Old Heirloom 1).

Heirloom 2 - Walk through the burial vault and off the ledge through the tree on the right. This should drop you behind the ghost wall that was below. Follow the path that goes behind the mausoleum where the two grave robbers are and keep going along the river that's on your left. The path eventually turns right and dead ends at a ghost wall and the second heirloom on the ground in front of it (Old Heirloom 6).

Heirloom 3 - Climb the hill to the right of the second heirloom and walk off it to bypass the ghost walls, returning to the area between the grave robbers and tour group. Run up the hill and go through the ghost buildings doorway. Head to the left, up the stairs and to the right to learn to teleport. Walk off the ledge and turn to the right. Walk through the openings in the ghost fences to a wall with a 'To Triage' sign above a hole. Teleport through the hole and once again follow the path through the fence openings. Head to the right at the tree and then the third heirloom can be found on a cot in the back left against the wall (Old Heirloom 5).

Heirloom 4 - From the third heirloom, turn to the right and head forward to teleport through the Exit opening. Follow the path beyond the head on the ground and remove the shaking ghost fence. Walk through to find the fourth heirloom laying on a grave (Old Heirloom 9).

Heirloom 5 - Turn around from heirloom four and walk around the tree to reveal a set of stairs. Walk up the stairs to find the fifth heirloom on the ground (Old Heirloom 8).

Heirloom 6 - Continue onto the next demon encounter and then head to the back of the area through the broken ghost wall. Turn left and the sixth heirloom will be on the ground to the right before the chainlink fence (Old Heirloom 13).

Heirloom 7 - The seventh heirloom is literally on the opposite side of the chainlink fence (Old Heirloom 2).

Heirloom 8 - Turn around and walk through the mausoleum next to heirloom six. Move passed the couple to see the eighth heirloom waiting to be revealed on the ground by a tree and chainlink fence (Old Heirloom 12).

Heirloom 9 - Turn around and walk through the mausoleums on your left to find the ninth heirloom sitting on a burial vault (Old Heirloom 7).

Heirloom 10 - Return to heirloom seven and follow the path down chasing the ghost girl. The 10th heirloom will be easily spotted on the path between many ghost tombstones (Old Heirloom 11).

Heirloom 11 - Walk forward to the two yellow shines be the tree to reveal the 11th heirloom at the base of the tree (Old Heirloom 10).

Heirloom 12 - After listening to Sophia, remove the ghost gate to the left of the tree. Walk through it and teleport to the opposite bank of the stream. Walk up just a little to find the 12th heirloom on the ground (Old Heirloom 5).
Note: If you've collected every artifact in the game up until this point, you should have just unlocked the following trophy upon collecting this heirloom:

Heirloom 13 - Continue along the path, teleport back over the stream, and further up the path you'll find the final Old Heirloom on the ground (Old Heirloom 4). Pick it up to unlock the trophy.

For this achievement, you must reveal 13 heirlooms in the Cemetery. When you locate them, follow the onscreen prompts. After you’ve collected all 13, the achievement will unlock and you can view and listen to a “ghost story” about these artifacts.

If you are obtaining all the collectibles in your playthrough, this will unlock naturally. Refer to “Collector All” for complete collectible video guides.
