To unlock this achievement you need to finish a combat using only mutations.

3- level up and unlock Dux mutation "SKULL SPLITTER"

Thanks for the guide! Just to clarify: do you only need to kill the last enemy with the mutation to get the cheevo? Skull splitter cannot cooldown after the first, I presume.

The video & solution is correct, but to be clear a "combat encounter" doesn't mean clearing a level -- picking off a lone guard where the game ends combat mode and puts you back into sneaking around mode will do.
So, play a new game as normal until you get XP to unlock Dux's Skull Splitter. Then kill a solitary guard silently with Skull Splitter and the cheevo will pop, no need to double-back as per the video. I played a new game normally and got it with the patrolling lone guard on Ark Outskirts.

The original guide will work perfectly but I wanted to add a "glitch" I found that seems to make this possible during a normal game.
I was near the end of the main game and farming kills for the "kill 10 people by electricity achievement".
I was fighting a lone target, and took him down to 5 health, and saved the game. I killed him then reloaded my save....the next time I killed him again (with electricity) and I got this achievement to pop even though most of his health had been taken off by guns.
It "appears" that the game only tracks damage done since the last save NOT since the start of an encounter.
IF that's the case then you can get the achievement any time, and don't need to start a new game and level Dux up a bit.
Would love it if some people can test this and confirm it works or not. Steps were:-
1. Attack lone enemy and take down to low health
2. Save and then kill enemy with mutation skill (I used lightning)
3. Reload save and re-kill enemy with mutation skill only
*achievement pops*

This is easiest done early in the game with a single 4HP ghoul. Use the Skullcrusher as soon as Dux get it and you will finish one of these off, a bit overkill but a safe way of getting this early and easy independent of difficulty you play at.

If you miss it early in the game, you can also get it by save scumming. Get an enemy to low health using whatever attacks, then save & reload and finish him off with a mutation skill and cheevo should pop, at least it did for me.
