MXGP 2019 - The Official Motocross Videogame

Before a race select 'Bike Setup', change any setting in any category then go back to the bike setup menu. Next select the bottom option (Bike Setup Management) and create a saved setup. The achievement will pop after you save the setup.

Note: You don't need to think about the setup change(s) you make as you can revert to defaults and/or delete the setup before racing.
