Chapter 11 You will be back at your house this is one of Six endings and you need to do all six for separate achievements.
First thing is to grab the bobby pin out of the garbage and head inside. Head upstairs grab the screwdriver off the little table and head back downstairs into the kitchen and into the bathroom before you open the trap door interact with the mirror and a little text thing will appear you need to enter your name ASHINA. After that use, the screwdriver on the trap door talk to the person down there and grab the shovel off the wall head back to the kitchen and interact with the fridge and then head out t the front of the house and use the shovel on the garden behind the garbage cans to get the knife. Head back inside and use the knife on the noodle lady by the stairs to kill her and head upstairs and use the bobby pin on your sissters bedroom. After some dialog, you will be in the Hospital and in control of your big sister you need to head out of the room and down to the main entrance of the hospital and use your pack of cigarettes on the can to the left of the main doors after your done head back inside and into your sisters room. After some dialog, the achievement will pop.

This is one of 6 endings available for the game. If you miss it on your initial playthrough, use Chapter Select and replay "Break or Strengthen".
After the section in the Train Station, you'll find yourself back home. Exhaust the dialogue with Sombria outside the house (you can skip the dialogue here). When you have control, inspect the trash cans on the left side of the house (you'll need to walk between the mailbox and house to get to them) to pick up a Bobby Pin. Go inside the house and up the stairs. Just to the right of Luzia's room (the room on the left) is a small cupboard. Interact with this to receive a screwdriver.
Head back downstairs and into the kitchen (entrance is just to the right of the stairs). You’ll meet the Noodle Lady here. Exhaust the dialogue (you can skip it) and head into the bathroom (the door on the top right). Exhaust the small dialogue here and use the screwdriver on the hatch leading to the basement to unlock it. Go down into the basement.
Here, you will find Mom. Exhaust the dialogue with her (you can skip it with
) and then pick up the shovel on the right. Head back up to the house but don't leave the bathroom yet. Instead, interact with the bathroom mirror. You will be prompted to spell a word. Put in ASHINA. When you’re done, exit the bathroom, through the kitchen, and into the living room of the house. Exhaust the dialogue with the Noodle Lady (you can skip this with as well). Go to the carrot garden on the left side of the house (get to it by going between the mailbox and house). Use the Shovel on the garden to receive a butcher’s knife. Head back inside the house. Use the knife on the Noodle Lady standing by the stairs. Exhaust the dialogue (you can skip it with ). When you’re done handling business, go upstairs and stand in front of the door on the right (Sombria's room). Use the Bobby Pin on the door to unlock it. Exhaust the dialogue.
When the dialogue is done, you’ll find yourself in a hospital and in control of Sombria. Exhaust the dialogue here (and with the Noodle Lady in the bed next to Luzia's) and exit the room via the bottom center of the screen. Head down, past two girls sitting on some chairs, past the front desk, and exit out the main doors of the hospital. On the left side of the doors, you'll see a smoking area with an ashtray. Stand next to this and open your inventory with
. Use the cigarettes with 
. Exhaust the dialogue and head back inside the hospital and back up into Luzia's room. Exhaust the dialogue until the credits roll and the trophy will unlock.
