during chapter 7 bus stop your sister will say she's hungry and vanish. Head into the woods to the right of the bus stop there will be a fork in the road take the right path. You will come to your sister eating a deer in a small clearing she will tell you she's hungry and you offer to go find some food. DONT DO ANYTHING!! you must wait for like a min or so and your sister will Die unlocking this achievement.

Chapter 7:
After you find your sister with the dead deer she will ask you for mushrooms. Save before you go for this achievement, because you'll get a game over screen. After that just wait and don't give the mushrooms to your sister.

This is missable during a playthrough but can be earned through Chapter Select by replaying the Chapter The Bus Stop.
After you trap the Witch in the Sauna, you'll take a ride on the back of a pickup truck and then find yourself at a bus stop. Shortly after arriving, you’ll be tasked with finding something for Sombria to eat. Instead of searching, just stand still. She'll banter you a bit with how hungry she is and how annoying you're being. Just keep waiting around and eventually she'll take matters into her own hands, unlocking the trophy.
Note: Earning this trophy will cause you to have to start the Chapter over again.
