In chapter 2 you need to have picked the brain after you interact with the therapist, she will turn into your sister leave the room and interact with the front door its locked enter the bathroom on the right side and take the hand out of toilet and try to leave the bathroom you cant and you will have a chat with your sister and unlock this achievement.
