This is earned at the start of Chapter 5 of Blood and Water. At the start of the level, you will be at the bathhouse waiting room. Head up through the double doors covered in blood and through the hole in the wall you made to get to the witch hanging on the wall. You will notice she is gone interact with her stand and turn around and leave the room. Slowly start walking right in the hallway till you see a face in the hole you just came out of. Head back inside and the witch will kill you earning this achievement.

Chapter 5:
Save before you go for this achievement. When you enter the bathhouse for the second time, go into the secret room for which you make a cut in the wall and read the note on the chair. After the event in the showers go back to this room to unlock this achievement.

Velvet
This is missable during a playthrough but can be earned through Chapter Select by replaying the Chapter Of Blood and Water.
After talking to the Noodle Lady in the laundry room, you'll need to go back to the Bath House. When you enter, exhaust the dialogue with Sombria and go through the door at the top of the lobby. Go through the hole you cut out in the wall earlier (far left side of the wall, it will be much bigger than before) and down to the end of the hallway. Exhaust the dialogue and leave this area. When you're back in the hallway, move a little bit to the right and you should see a white face appear in the hole you just came out of. Go back inside the hole and a cutscene will play, unlocking the trophy.
Note: Earning this trophy will cause you to restart the chapter.
