This is a story related achievement during chapter 8 you will be on a train. a little way into the level after you meet an old friend you will be in the rear of the train. Start running towards the front of the train and you will run into the noddle lady again after you talk to her this will unlock

You'll also earn this in the Cold Rails Chapter while riding on the train. After you unlock the Luggage Carriage with the help of Little Jacket, explore the very back of the train (make sure you go out the back door). You'll go through some dialogue and when you regain control, start making your way back to the front of the train (to the Engine room). As you pass through the large passenger carriage, you’ll be stopped by the Noodle Lady. Exhaust the dialogue with her and the trophy will unlock.
