Simply finish the game completely on Survival Difficulty to gain this achievement (you can't leave early, or use Free-cam Mode and go to the final room in the hotel and leave). Lower difficulty achievements will also unlock when you get this.
I'm not sure if modes Cancel out achievements or not, but on my 3rd playthrough I used the following cheats:
Infinite Ammo
Mega Wrench
Speedy Gordon
Fast Weapons
Invincibility
And was able to mop up the achievements I had missed previously, and other people have reported they also got achievements with these cheats on, So you can get this with these cheats enabled.
Note: Cheats are "found" by collecting tapes in the world, and completing the game on the harder difficulties but are not unlocked until you complete a full playthrough of the game.
