Shortly after you disconnecting the antenna you will grab a flashlight and enter an arena with a many headed puppet.
Pull the switch on the wall and dodge the monster while waiting for the gate to slowly open.
The achievement will pop when you complete the game as long as you didn't kill... them.
Alternatively: You can start a new game in Free-Cam Mode. enter the building as you normally would, but go up stairs and enter room 121 and then leave. leaving the building will trigger the end of the game, and since you didn't kill the boss, the achievement will pop.
