Saving in the game is done by using a token in the Green Public Savestation's in the game.
Simply finish the game completely without using a save booth to gain this achievement (you can't leave early, or use Free-cam Mode and go to the final room in the hotel and leave).
I'm not sure if modes Cancel out achievements or not, but on my 3rd playthrough I used the following cheats:
Infinite Ammo
Mega Wrench
Speedy Gordon
Fast Weapons
Invincibility
And was able to mop up the achievements I had missed previously, and other people have reported they also got achievements with these cheats on, So you can get this with these cheats enabled.
Note: Cheats are "found" by collecting tapes in the world, and completing the game on the harder difficulties but are not unlocked until you complete a full playthrough of the game.
